A consent form does two jobs. It screens for the things that make a service unsafe, like blood thinners before a tattoo, retinoids before waxing, or an adhesive allergy before lash extensions. And it records that the client understood the risks and agreed to aftercare before you started. If a client later has a reaction they did not tell you about, the signed form shows what they disclosed and what they agreed to. The strongest forms ask clients to initial each statement rather than sign once at the bottom, because it proves every line was read. Keep it short, specific to your service, and stored for as long as your local rules require.
